Full assistance with washing and dressing in the morning; prefers not to be rushed. Prefers her blue cardigan. Female carers only.

Mobility & falls

High falls risk (recent unwitnessed fall). Uses a frame. Encourage the hallway to be kept clear; loose rug being removed. Assist to garden bench as a daily goal.

Nutrition & hydration

Diabetic diet. Appetite currently low — prompt and encourage; target 1,500ml fluids/day. Tea with one sugar, milk in first.

Medication

Self-administers with prompting for morning and lunchtime; carer administers evening. See MAR below. Allergy: penicillin.

Communication & wellbeing

Early vascular dementia — approach calmly, one instruction at a time. Enjoys Classic FM and talking about her garden and late husband Albert.
